It must've been created in a program that adds the icon. I'm sure there's some freeware or shareware app that would do the same thing for you via drag and drop if that's what you want.

Try resetting your PRAM. Shutdown, then startup while holding (requires some finger stretching) Command-Option-P-R See if this helps.
